A mother-of-four from Streetly died after a nurse at a trouble-hit hospital trust gave her ten times the amount of drugs she was supposed to receive.
Arsula Samson, 80, had a heart attack at Good Hope Hospital, Sutton Coldfield, after she was given an overdose of deadly potassium chloride.
